Please Note: Each household must complete a household Application Form and individual Waiver Forms. You will submit them to your applicable coordinator (Stockholder's - Ken Ashe) / Associate's - Michelle Dorr). You may digitally submit forms via email, or print and submit them directly at your Pool Safety Training. Pool keys will not be given out unless forms are received and the Pool Safety Training is completed.
